In this guide, you'll learn to add location and activity information for index patients during an outpatient encounter for a communicable disease case.
1. Adding location and activity information for Index Patients

- Go to the Location and Activity sub tab to begin documenting the location details.

- Confirm the location and activity is required by selecting Yes.

- Add a new location or activity for the patient by selecting Add.

- Add the location where the index patient was present during possible exposure to the communicable disease.

- Include the date and time that the index patient was present in the location or participated in the activity.

- Enter the length of time that the patient was present. Use the format hour and minute for the hours or minutes the patient spent there.

- Enter the amount of people, excluding the case, who were present.

- Add the setting type, such as indoor or outdoor, noting whether ventilation was available.

- Enter the speech volume of the people present. Indicate whether conversations were low, normal, or loud from the dropdown menu.

- Include the physical distancing of the people present. Indicate from the dropdown menu if they were distanced at all times, most of the time, not at all, or if unknown.

- Indicate whether the index case and the people present were wearing masks.

- Select the Transmission Risk.

- Based on the provided information, indicate the transmission risk level for the index case. For example, using the table, no face covering but outdoors and ventilated can indicate a low risk. Select Low on the table for these sections, then close the table.

- The transmission risk has been added automatically from the selection you chose.

- Click Save.

- Click Analyze after making all entries for the index case.
